CHRONIC DISEASE ANNUAL REVIEW

If you have one or more of the following conditions we will carry out an annual review, usually, in the month of your birth.  

Type II Diabetes; Hypertension; Chronic Kidney Disease; Heart Disease; Thyroid Dysfunction; Stroke; Peripheral Vascular Disease; Pre-Diabetes.

In order for us to carry out this review correctly, please follow these steps to ensure we have all the information required.

You will receive either a text or a letter from us informing you that your review is due.  Depending on your condition(s) we will

need all or some of the following before the nurse can see you. 

 

    • Blood Tests

    • A urine sample. Please collect from reception at the practice a white top sample bottle.  You should provide us with your first urine specimen of the day.

    • Seven days of 2 sets of blood pressure readings taken at home. If you do not have your own BP monitor you may be able to borrow one from the surgery.  A £10 deposit will be payable upon collection and will be returned when you bring the monitor back to the practice.



 Please note, our nurses require the results of all of the above tests to be returned to them before they can see you.  Please only book a follow-up nurse appointment once you have completed all of the above.
