Changing your contact details

To update details like your name, address, phone numbers or email address:

• fill out a change of contact details form
• phone or visit the surgery from 8:30am to 5:30pm

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

Our practice area includes Bannockburn, Braehead, Broomridge, Cambusbarron (restricted by boundary), Cambuskenneth, Causewayhead (restricted by boundary) Cornton, Kildean (restricted by boundary), Kings Park, Raploch, Riverside, St Ninians, Torbrex, Town Centre, Whins of Milton

Please ensure you tell us if you change your phone number or email as we may need to contact you from time to time.  The practice may contact you via text message regarding non clinical matters, if you are not happy to be communicated with in this way please let a member of staff know your preferences.

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
