Registering with a GP practice

To register permanently, you’ll need to complete a registration form. You can either:

  • Download a registration form, complete it, and then email it to the GP practice fv.gp25737viewfieldadm@nhs.scot or hand it in at reception.
  • Registrations will normally be completed within 24 hours after which time you will be able to access GP services.  If you need care before this time please speak to the reception team.

When you register with us, as well as your personal details you will be asked to provide information on your previous address, GP practice, your mother's maiden name and your place/country of birth.  This will help us to trace your previous records.  Patients moving from outwith Scotland may find that it can take up to several months for other Health Authorities to transfer full records.  Unless you bring them with you (fully translated) we will not receive medical records from outwith the UK.

 

 

  

Registering as a Temporary Resident

You can only permanently register with a GP practice if you’re staying in the area for more than 3 months. If you’re not, you may still be treated but as a Temporary Resident.

The first practice that you attend MUST register you as one of their temporay patients, whether they are accepting permanent patients or not.  Each practice may offer different appointment opportunities.  Please speak to the receptionist to find out further details of how and when you will be able to be assessed by a clinician.  

Anybody in Scotland can access primary care services at a GP practice without charge.

Accessing Medical Information or Copies of Medical Records

Confidentiality is taken very seriously and, therefore, full consent must be gained from the registered patient before any details can be shared with a third party (eg Insurance Company, Solicitors or other family members). 

If you wish to request the whole or part copies of your medical records this should be made at reception where a form will be provided for you to complete.  Provision of records can take up to 28 days to process.  No charge is applied for the first issue of records but a further request for the same records covering the same period will be chargeable.  You will be asked to collect and sign for the records once they are ready.

Request for copies of records for patients who are deceased should be requested from the Health Board as special conditions for their release will apply.
