If you’ve been off sick for more than 7 days

To request a sick note:

  • phone or visit the surgery from 8:30am to 12.30pm and 1.30pm to 5:30pm.  Fit notes are usually issued within 5 days of the request being received and can be backdated.
  • Please note, if you have attended a clinic, been an inpatient or been to MIU/A&E and they have advised that you are unfit for work, the responsibility for the issuing of the fit note lies with that clinical team and not with the GP practice.

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
